What Is
Remote Patient Monitoring (RPM)
Remote Patient Monitoring (RPM) is a service that regularly and frequently collects, stores, and displays a growing archive of clinical patient (resident) data.
Practitioners have 24/7 access to the information and use it to improve the treatment and management of patients with chronic health conditions like hypertension, diabetes, or COPD.
Devices include common medical devices such as blood pressure cuffs, glucose monitors, or weight scales. All the patient (resident) must do is use the devices per practitioner's advice.
What Does RPM cost the patient (resident)?
The positive impact on patient (resident) health, safety, and quality of life in RPM directly lowers the total cost of care.
As a result, the service is reimbursed by payers, including Medicare and Medicaid. There is no cost (including copay) to the patient (resident) enrolled in the service.
Remote Patient Monitoring Benefits
What can RPM track?
Remote patient monitoring can be used to track the following symptoms and conditions
How to start the RPM service?
The process is already in place. All a patient (resident) needs to do is contact their medical practitioner and express a desire (consent) to begin the service.
In some cases, a practitioner may recommend RPM to improve the treatment of a patient’s (resident’s) chronic condition.
In either case, once the patient (resident) consents to start RPM with a practitioner, the service will be ordered, configured, and installed, including training with the patient (resident).
